Home Care

Home Health is care provided in the
home by one or multiple skilled professionals.

These professionals could consist of a nurse, physical therapist, occupational therapist, speech therapist or a social worker who meet, discuss and work together as a team to treat an illness or injury with the goal of keeping the patient out of the hospital, regaining their independence and becoming as self-sufficient as possible.

At Las Cruces Comprehensive Rehab and Home Care we offer a unique type of service. Our focus is on continuity of care. Once admitted into our home care program you will be followed by a Registered Nurse or Physical Therapist who work in the outpatient clinic as well as in the home. This allows them to be fully aware of your needs when the time comes to transition from home to the outpatient clinic setting new goals consistent with your progress gained while in the home setting.

• Monitoring serious illness and unstable health status
• Skilled Nursing Care
• Wound care

• Patient and caregiver education
• Infusion or Nutritional Therapy
• Medication Management
• Physical Therapy
• Occupational Therapy

Who Pays for Home Care?

Home care is paid for by a variety of different payers. In many cases, there is no out of pocket expenses to the beneficiary.
